About

Ontario Health is seeking a palliative care health service provider for the role of Clinical Lead, Models of Palliative Care: Hospital. The Clinical Lead will provide strategic clinical leadership and evidence-informed advice to support implementation and adoption of the palliative care model of care for adults receiving care in hospital settings, and will lead knowledge translation, stakeholder engagement, change management, implementation planning, workforce considerations, evaluation, and development of recommendations that respond to emerging system priorities. This work will advance the Health Services Delivery Framework and the goals of the Ministry of Health’s Provincial Framework for Palliative Care.
